| Dr. Lee’ research interest has focused on the molecular mechanism of obesity and insulin resistance. Obesity is associated with an array of health problems including insulin resistance and type 2 diabetes, fatty liver disease, atherosclerosis, and hypertension. However, these diseases are not associated with obese patients in 10-30% of cases. This is referred to as ‘metabolically healthy obesity’. It is now clear that the adipocyte is not a simple energy store but rather an endocrine gland playing an important role in fuel metabolism. Adiponectin is one of the adipocyte hormones (adipocytokines) that has many favorable effects on metabolism, including improvement of insulin action and reduction of atherosclerotic processes. Unlike other adipocytokines, the plasma level of adiponectin is reduced in obese individuals. Recently, his lab reported that mitochondrial function is essential for adiponectin synthesis in adipocytes, and mitochondrial dysfunction in adipose tissue explains decreased adiponectin synthesis in obesity. He will present his recent data on the role of mitochondrial function in adiponectin synthesis and its implications in the genesis of metabolically healthy or unhealthy obesity. |
| 울산대학교 의과대학 이기업 교수는 당뇨병 및 비만증의 발병 기전과 한국인 당뇨병의 특징에 관해 깊은 관심을 가지고 연구를 수행해 왔다. 비만이 당뇨병이나 지방간, 고혈압, 동맥경화증 등 여러 질환의 공통적 발병 기전임은 널리 알려진 사실이다. 그러나 10-30%의 비만 환자에서는 이와 같은 질환이 동반되지 않아 이를 ‘건강한 비만; metabolically healthy obesity’라고 지칭하고 있다. 체내 잉여 에너지를 저장하는 창고로 알려진 지방세포에서 여러 가지 호르몬이 생산됨이 알려졌는데 최근 이중에서도 당뇨병이나 동맥경화증 발생을 억제하는 기능을 가진 adiponectin에 대한 관심이 증가하고 있다. Adiponectin의 혈액 내 농도는 다른 지방세포 호르몬과는 달리 비만 환자에서 감소되어 있는데 최근 이기업 교수팀은 이것이 지방세포 내에 있는 미토콘드리아 기능과 연관됨을 밝힌 바 있다. 본 강의에서는 ‘metabolically healthy obesity’라는 새로운 개념에 대한 설명과 최근까지 진행된 연구 결과를 발표하고자 한다.

| Diabetic cardiomyopathy has been defined as ventricular dysfunction that occurs independently of coronary artery disease and hypertension. Dr. E. Dale Abel, of the University of Utah, United States of America, has spent over 20 years studying and experimenting on the molecular mechanisms of cardiac dysfunction associated with obesity and diabetes. His lab is focused on understanding the molecular mechanisms that are responsible for cardiovascular complications of diabetes and have focused primarily on the role of mitochondrial dysfunction in the diabetic cardiomyopathy. Autophagy is a self-digestive process that degrades the cytoplasmic constituents. In this lecture, he will also review novel mechanisms by which increased myocardial lipid uptake impairs autophagosome turnover and review the implication of these phenomena in the pathogenesis of mitochondria dysfunction that contributes to the pathogenesis of diabetic cardiomyopathy. |
| 당뇨병성 심근병증은 당뇨병환자에서 발생하는 심실기능저하로 정의할 수 있다. University of Utah의 E. Dale Abel 교수는 20년 이상 비만 및 당뇨에서 나타나는 심장 기능이상의 분자생물학적 기전에 대한 연구를 수행하였으며, 특히 미토콘드리아 기능 이상에 대해 깊은 관심을 가지고 연구를 수행하고 있다. Autophagy는 세포 내에서 일어나는 자기소화과정으로 오래된 단백질이나 세포질 내 구성요소의 전환에 관여한다고 알려져 있다. 본 강의에서는 당뇨병성 심근병증의 발생과정에서 미토콘드리아 기능 이상의 중요성에 대한 최근까지의 연구 결과 및 심장 근육에서 인슐린 신호 전달체계가 autophagy를 조절하는 기전 및 그 의미를 설명하고자 한다.
