: Elderly diabetes

There is a lot of major concern surrounding the medical problems of the elderly, such as ‘rapid aging’ or ‘problems of the elderly’. Additionally, among such diseases, the burden from diabetes is the greatest, and interest regarding its medical, social, and economical aspects is growing. Management of diabetes for the elderly requires an interdisciplinary approach which includes geriatrics, diabetes, education, epidemiology for the elderly, and nutrition. The first lecture will be given by the president of our organization, Dr. Hyung Joon Yoo of Hallym University, Korea, and is entitled ‘geriatric secrets’. The secrets associated with characteristics and quality of life will be unraveled. The second lecture by Dr. Hak Chul Jang of Seoul National University, Korea is entitled ‘Geriatric syndrome and elderly diabetes’. This lecture focuses on how age is commonly accompanied by important risk factors such as muscle thrombocytopenia, senility, and so called elderly syndrome, and also focuses on prevention and treatment of elderly syndrome. The third lecture, entitled ‘Diabetes in old age’, will be presented by Hong Woo Nam, the section chief of National Medical Center, Korea, , which will outline the associations and differences between elderly diabetes and typical adult diabetes. The final lecture entitled ‘Education of the elderly with diabetes’ will be delivered by Myeong Hee Hong, a professional nurse specializing in diabetes education from the Seoul Paik Hospital, Korea, which will outline social, mental, clinical, and functional conditions– discussion on how to plan for numerous aspects that should be considered in the education for diabetes for the elderly will be made.
급속한 고령화와 노인문제로 인해 노인의 의학적 문제가 중요한 관심사가 되고 있다. 특히 단일 질환 별 질병부담이 1위인 당뇨병에 대한 의학적, 사회 경제적 관심이 증대되고 있다. 노인당뇨병 관리는 노인병학, 당뇨병학, 노인역학 및 영양학 등 다학제적 접근이 절대적으로 필요한 분야라 할 수 있다. 첫 번째 강의는 본 학회 회장이신 한림의대 유형준 교수가 ‘Geriatric secrets’의 제 목으로 노인병의 정의, 특성과 삶의 질과 연관된 비밀들을 풀어나갈 것이다. 두 번째 강의는 서울의대 장학철 교수가 ‘Geriatric syndrome and elderly diabetes’의 제목으로 당뇨병환자가 나이가 들어감에 따라 흔히 동반되는 근감소증과 노쇠 등 소위 노인증후군의 중요한 위험인자들과 노인증후군의 예방 및 치료에 대해 강의해 주실 예정이다. 세 번째 강의는 국립중앙의료원 남홍우 과장이 ‘Diabetes in old age’의 제목으로 노인당뇨병의 특성과 일반 성인당뇨병 관리와 달리 특별히 고려하여야 할 사항에 대해서 강의해 주실 예정이다. 마지막 강의는 서울백병원 홍명희 당뇨병 교육전문 간호사가 ‘Education of old persons with diabetes’의 제목으로 사회적, 정신적, 임상적, 기능적 상태뿐만 아니라 당뇨병의 상태가 다양한 노인에서 당뇨병 교육 시 고려해야 할 점과 어떻게 계획해야 할지에 대해서 강의해 줄 예정이다.

: Outcome evaluation of diabetes education

Each year, the number of diabetes patients increases, and the financial burden placed on the treatment of diabetes also concomitantly increases. Emphasis is being made on the importance of education in diabetes management. However, only approximately 20 to 30% of diabetes patients receive proper diabetes education. Additionally, the cost and analysis of overall health effects of diabetes education has not been compiled, and the public recognition of the cost of diabetes education, as well as the support and implementation of the nationwide diabetes education program, is insufficient. In this session, we will cover the cost-effectiveness of diabetes education. Short-term performance, performance measurement, and assessment tools are at the core of diabetes education. The effectiveness of diabetes education and 7 instantly noticeable behavioral habits that can be evaluated will be examined. In order to determine the economical feasibility of utilizing a diabetes-specialized nurse, Jin-Won Noh of the Eulji University, Korea, assessed a methodology for measuring the direct or indirect effects of results from diabetes program funding, which was assessed with a diabetes education medical outcome model (health outcomes model) and nursing outcome model. Hee Sook Kim from Seoul National University, Korea, will provide a lecture on the evaluation process. Jeong Mi Lee from the Gwangmyung Sungae Hospital, Korea, will take the time to introduce the standard guidelines and assessment tools for the AADE7 self-care behaviors, which are the main diabetes management factors evaluated by Jin Hee Jung from the Seoul National University Bundang Hospital, Korea. Through this session, an understanding of the effects of diabetes education will be raised, and methods of effective evaluation of education will be discussed.
당뇨병환자 수가 해마다 증가함에 따라 당뇨병 치료에 소모되는 사회적 비용도 증가하고 있다. 당뇨병 관리에 있어 당뇨병 교육의 중요성은 강조되고 있지만 당뇨병 교육을 제대로 받는 환자 수는 20-30%에 불과하다. 또한 당뇨병 교육에 대한 비용 및 건강 전반에 대한 효과분석도 이루어지지 않고 있으며, 당뇨병 교육비에 대한 사회적 인식과 국가적인 당뇨병 교육 프로그램의 지원과 시스템 구축도 부족한 상태이다. 이에 본 세션에는 당뇨병 교육의 비용 효과, 장단기 성과, 성과 측정 방법, 평가 도구와 당뇨병 교육의 핵심이며, 당뇨병 교육 효과를 즉각적으로 평가할 수 있는 7가지 행동습관에 대하여 살펴보고자 한다. 을지대 의료경영학과 노진원 교수는 당뇨병 전문 간호사의 당뇨병 교육의 경제적 타당성을 파악하기 위하여, 당뇨병 교육 프로그램에 투입된 비용과 그 결과, 그에 따라 나타나는 직•간접 효과를 측정하는 방법론에 대하여, 당뇨병 교육의 의료성과 모델(health outcomes model)과 간호성과(nursing outcomes)를 측정하고 평가하는 방법에 대해서는 서울대 김희숙 교수가 강의해 줄 예정이다. 광명성애병원의 이정미 선생은 당뇨병 관리의 핵심요소인 AADE7 self-care behaviors를, 분당서울대병원 정진희 선생은 당뇨병 교육의 효과 평가를 위한 표준지침과 평가 도구에 대해 소개하는 시간을 갖고자 한다. 이 세션을 통해 당뇨병 교육 효과에 대한 이해를 높이고 효과적인 평가 방법을 배우는 기회가 되기 바란다.

: Advanced glycation endproduct(AGE), sodium intake and
nutrition in type 2 diabetes

Solving the problem of the rising mortality rate due to diabetic vascular complications is challenging. The occurrence of vascular complications is mainly the result of advanced glycation end-products. In this session, 4 distinguished Korean doctors and scientists in regards to vascular complications and advanced glycation-end products, have been selected. The first lecturer will be professor Hyun-Sun Lee, who has performed a large amount of research on nutrition and advanced glycation end-products. He will be presenting ‘Advanced glycation end-products (AGEs) and nutrition in people with type 2 diabetes’. The second lecturer will be professor Chong Hwa Kim from Sejong General Hospital, Korea, who will be presenting the clinical importance and meta-analysis of study of sodium intake in diabetic patients. The third lecturer is professor Jung Sug Lee from Food and Nutrition Science Alliance, who has performed many nutrition-related statistical analyses, who will be presenting ‘How to measure sodium intake & use of KNHANES data in people with diabetes mellitus’. The final lecturer will be professor Jung Eun Lee from the food and nutrition department of Sook Myung Women's University, Korea, who will be presented about salt intake in Korean diabetes patients and the National Health and Nutrition Examination Survey. This will be very helpful in understanding the causes of diabetes and vascular complications, such as advanced glycation end products, in association with sodium.
당뇨병환자에서 혈관합병증 발생으로 사망률이 증가하는 문제는 해결하기가 쉽지 않은 과제이다. 혈관합병증 발생에 주요한 원인으로는 최종당화산물, 나트륨 등이 있다. 이번 심포지엄에서는 혈관합병증과 최종당화산물, 나트륨에 관해서 국내 저명한 선생님들 4분을 모시고 알아보고자 한다. 첫 번째는 영양과 최종당화산물에 대한 많은 연구를 하신 고려대 식품영양학과 이현순 교수를 모셔서 ‘Advanced glycation endproducts (AGEs) and nutrition in people with type 2 diabetes’ 주제로 강연을 듣고, 두 번째는 당뇨병환자에서 소금섭취의 임상적 의미와 메타분석을 통한 소금섭취의 중요성에 대해 세종병원 김종화 과장이 발표한다. 세 번째는 영양관련통계를 많이 분석하신 FANSA 이정숙 박사를 모시고 ‘How to measure sodium intake & use of KNHANES data in people with diabetes mellitus’ 주제로 강연을 듣고, 마지막으로 숙명여대 식품영양학과 이정은 교수를 모셔서 한국인 당뇨병환자에서 소금섭취량과 국민건강영양조사 자료를 이용하여 국내 당뇨병환자의 소금섭취량 연구방법에 대해서 알아보려고 한다. 대한당뇨병학회 많은 회원 분들이 참석하여 당뇨병과 혈관합병증 원인인 최종당화산물과 나트륨에 관한 이해를 돕는 시간이 될 것으로 기대한다.

: The secret power of physical activity

A common knowledge is the fact that routinely activity lowers the susceptibility of forming diseases such as hypertension, coronary artery disease, stroke, diabetes, breast cancer, colorectal cancer, depression, and others. Additional benefits are well toned body and physical energy balance. Though these profits from physical activity are widely known, how they emanate is controversial. The focus of this discussion will be the approaches on type 2 diabetes treatment correlating it with obesity and exercise.
규칙적인 신체활동은 고혈압, 관상동맥질환, 뇌졸중, 당뇨병, 유방암, 대장암, 우울증 등 다양한 질환의 발병위험을 감소시키는 것으로 잘 알려져 있다. 또한 인체 에너지 균형과 체중조절의 핵심이 되고 있다. 신체활동의 유익한 효과는 잘 알려져 있지만 어떻게 그런 효과를 나타내는지에 대해서는 여전히 논란의 여지가 남아있다. 본 세션에서는 제2형 당뇨병, 비만, 그리고 운동 간의 상호 연관성을 바탕으로 제2형 당뇨병의 치료전략을 토론하고자 한다.
